West Sacramento Nursing and Rehabilitation Center is a CIHQ accredited Congregate Living Health Facility that serves the greater Sacramento Metropolitan area and surrounding communities. Our community-based setting is ideal for residents with complex medical needs and features walking paths, gardens, fountains, and a beautiful courtyard that makes the West Sacramento Nursing and Rehabilitation experience comfortable and serene. Our recently constructed facility offers a bright and modern environment that is therapeutic and conducive to healing. We proudly serve and care for our residents to help them return to their communities whenever possible. We look forward to meeting you!

Medical Supervision
Provided by a physician-lead interdisciplinary team of nurses, four different types of therapists, a registered dietitian, and other specialists to ensure the best and most appropriate care is given.
Skilled Nursing
24-hour, sub-acute care on a recurring, intermittent, extended, or continuous basis by highly trained nurses who specialize in treating prolonged, medically complex illnesses and injuries.
Social Engagement
Offering social and recreational activity programs to create an uplifting, engaging environment where our residents can enjoy valuable social interaction and mental engagement.
Terminal Patient Care
Assisting both residents and their families during transitional periods and providing them with the information and support after a written diagnosis by an attending physician or surgeon.
