Services
At West Sacramento Nursing and Rehabilitation Center we offer services that support and address the needs of every patient. We believe that recovering successfully from brain, spinal cord, and medically complex injuries, illnesses, and other challenges is a process and we have developed an effective and flexible continuum of sub-acute and post-acute care that helps guide our residents through their rehabilitation journey. Our plan of care ensures an ongoing focus on the resident’s well-being and advancing toward the goal of returning to day-to-day living in their homes or community.
We provide skilled nursing care on a recurring, intermittent, extended, or continuous basis, letting the resident’s needs prompt our course of action. This care is generally less intense than what is offered in acute care hospitals but more comprehensive than what is provided in a skilled nursing facility.

- Spinal cord injuries – all levels
- Complex wound care – including VAC therapy
- Pulmonary – including vent care and weaning, trach care, suctioning, and decannulation
- Neuromuscular disorders – including Guillain-Barre, Muscular Dystrophy, Myasthenia Gravis, and ALS
- Medically complex cases – including multiple trauma/fractures, multiple IV antibiotic therapy, and G- and J- tube feedings
- Acquired brain injuries – traumatic, anoxic, aneurysms/bleeds, cerebral vascular accidents
- Orthopedically complex cases – including multiple trauma/fractures
- Rehabilitation – including licensed physical, occupational, respiratory, and speech therapists, social services, and community integration
